ADMISSIONS PROCESS

We welcome your interest in our programs and we would love to sit down with you to discuss your plans with us.

In general most individuals who choose to join our programs have already enrolled in the Florida Home and Community based Waiver program. The Agency for Persons with Disabilities (APD) can assist you in enrolling in this program where you will be able to choose what waiver services you would like to receive at no cost to you. You will also have the opportunity to select a Waiver Support Coordinator (WSC) who will work with you to select services and providers such as Bridges and develop a cost plan with you.

Once you have enrolled in the waiver program and you have selected Bridges for some or all of your services please contact us at 321-690-3464 (ext 18) to arrange a convenient time for you to meet with us. We will sit down with you and discuss each service with you and ask you to complete an admissions package. You will also have the opportunity to discuss what you would like to accomplish as we start to develop your support plan.

Vocational Rehabilitation

Contact your local VR office if you have an interest in seeking employment either in the community or our AbilityOne program. You will be assigned a VR Counselor who will outline your options and list local providers who offer employment and supported employment services including Bridges.

Supported Employment (waiver service)

However if you are interested in Supported Employment through the Waiver, you should discuss this with your WSC who will discuss your options with you.
 

HOME AND COMMUNITY BASED WAIVER

Please visit the APD website for a comprehensive review of this waiver including qualifications, eligibility etc.

Please be aware that the earlier you are qualified for waiver services, the more options you and your family will have. It is not unusual for families to begin to explore this process during the elementary and middle school years. There is a growing statewide waiting list for waiver enrollment and there are limited slots allocated throughout the state.

Also, in response to legislative changes in law, APD recently implemented a 4-tiered waiver program.  APD staff reviewed cost plans for all individuals currently enrolled in the Developmental Disabilities and Family and Supported Living Waivers, and the Consumer Directed Care program and assigned each individual to a tier waiver based on criteria specified.

These actions have tended to reduce the number of available choices for individuals who are currently enrolled in the waiver. Therefore starting the waiver process early is highly recommended.
