How do I sign up for these services?

Each of these services is available through the Home and Community Based Waiver Program (HCBW) coordinated by The Agency for People with Disabilities (APD), State of Florida. Waiver eligible individuals and their families will have the opportunity to select an independent support coordinator who can assist them in choosing which provider(s) can offer the best combination of services to them.

Bridges offers a full-service resource center which can guide you through this process and answer all of your questions. We assist with the admissions process, development of your individual support and cost plans, and liaison with your chosen support coordinator. We also assist you with transportation options as well as information about other programs in Brevard Country including social, civic, and recreational opportunities.  If needed, we will advocate on your behalf for other services you require.
